On the evening of March 24, 2025, a tragic accident occurred at a distributed photovoltaic construction site in Dahua Yao Autonomous County, Hechi City, Guangxi. Worker Wei Moumao was using an angle grinder without a protective guard when the cutting disc suddenly shattered. A fragment struck his neck, severing an artery. Despite emergency treatment, he died an hour later from massive blood loss.
This seemingly minor oversight — operating without a guard — revealed deeper systemic issues in industrial safety.
01 The Accident in Focus: A Chain of Failures
According to the official investigation report released by the local government on August 12, 2025, the accident resulted in one death and economic losses totaling 1.03 million RMB. The report confirmed that Wei Moumao was cutting steel square tubes on a rooftop when the disc broke apart due to high-speed rotation and lack of a safety guard.
Witnesses described hearing a sudden scream before seeing the grinder drop and Wei clutching his bleeding neck. Two deep wounds were found on the left side of his neck. Although first aid was administered quickly, it was too late.
02 Root Causes: Not Just One Man’s Mistake
The direct cause was Wei’s improper use of the grinder. But the underlying problem was a failure at every level:
The construction company did not provide safety training or ensure proper use of protective equipment.
No emergency plan was in place.
Labor protection gear was missing or substandard.
Supervisors failed to enforce safety rules.
Angle grinders rotate at high speed. Without guards, shattered discs become deadly projectiles. Safety manuals universally warn: “Do not use grinders without safety covers.” Yet, this basic requirement was ignored.
03 Oversight Breakdown: Superficial Inspections, Zero Impact
Shockingly, the local Development and Reform Bureau had conducted nine inspections between 2024 and early 2025. Yet none of these discovered the safety risks present at the site. The investigation criticized the lack of technical competence and shallow enforcement.
Even worse, companies involved refused to cooperate with the investigation, withholding documents and dodging questions. This not only obstructs the investigation but highlights a broader culture of noncompliance in the industry.
04 The Lessons: Systemic Risks Require Systemic Change
The investigation report called for a comprehensive rectification:
Stricter project vetting: Photovoltaic project approvals must meet national safety and compliance standards.
Enforce training and certification: Operators must pass exams and hold valid qualifications before using cutting or grinding tools.
Real inspections, not just paperwork: Safety oversight must focus on actual conditions, not formal checklists.
Detailed safety protocols already exist:
Always wear protective glasses and helmets.
Never stand in the line of the rotating disc.
Only operate from the safe side of the grinder’s cover.
Use matching disc sizes and reject damaged or expired discs.
But these rules are too often neglected on fast-paced job sites.

Angle grinders are common — and deceptively dangerous. Behind every accident lies a string of ignored warnings, skipped trainings, and overlooked checks. Safety is not a formality. It is a lifesaving discipline.
This tragedy is not just about one broken cutting disc. It's a wake-up call for the entire industry.
